The term “angioplasty” means using a balloon to stretch open a narrowed or blocked artery. However, most modern angioplasty procedures also involve inserting a short wire-mesh tube, called a stent, into the artery during the procedure. The stent is left in place permanently to allow blood to flow more freely. Skilled Iranian cardiologists use modern techniques to perform effective angioplasty procedures with the best results. While the surgery is extremely successful, it can cost a large sum. To decrease the costs, many patients opt for getting angioplasty abroad in affordable countries. Among all the medical destinations one can undergo angioplasty in; Iran is favorable destination for a large number of people.
Coronary angioplasty is sometimes known as percutaneous trans luminal coronary angioplasty (PTCA). The combination of coronary angioplasty with stenting is usually referred to as percutaneous coronary intervention (PCI).
When a coronary angioplasty is used
Angioplasty is a common medical procedure in Iran and Iran is ranked among the top 10 countries in the world for cardiovascular surgeries. It may be used to:
- Improve symptoms of CAD, such as angina and shortness of breath.
- Reduce damage to the heart muscle from a heart attack. A heart attack occurs when blood flow through a coronary artery is completely blocked. Angioplasty is used during a heart attack to open the blockage and restore blood flow through the artery.
- Reduce the risk of death in some patients.
When to Consider Coronary Angioplasty
Your doctor may recommend angioplasty if:
- You have chest pain or shortness of breath due to CAD.
- You have significant narrowing or blocking of only 1 or 2 coronary arteries. Your doctor may recommend heart bypass surgery (coronary artery bypass graft surgery) instead of angioplasty.
- You have had a heart attack.
- You are not feeling better despite medicines and lifestyle changes to reverse atherosclerosis.
- You have a coronary bypass graft that has closed or narrowed.
Who Should NOT Have Angioplasty?
You should not have angioplasty if:
- You have mild to moderate narrowing of the arteries and no symptoms.
- Your condition is responding well to medicine and lifestyle changes.
People with kidney disease or reduced kidney function may not be good candidates for angioplasty. During angioplasty, doctors inject a contrast dye into your bloodstream in order to see (by x-ray) where the blockages are and to guide treatment. Because the kidneys have to filter the dye out of your body, people with kidney problems have a greater risk of kidney-related complications from angioplasty.
What Are the Risks of Coronary Angioplasty?
Coronary angioplasty is a common medical procedure. Although angioplasty is normally safe, there is a small risk of serious complications, such as:
- Bleeding from the blood vessel where the catheter was placed.
- Damage to blood vessels from the catheter.
- An allergic reaction to the dye given during the angioplasty.
- An arrhythmia (irregular heartbeat).
- The need for emergency coronary artery bypasses grafting during the procedure (two-four percent of people). This may occur when an artery closes down instead of opening up.
- Damage to the kidneys caused by the dye used.
- Heart attack (three-five percent of people).
- Stroke (less than one percent of people).
As with any procedure involving the heart, complications can sometimes, though rarely, cause death. Less than two percent of people die during angioplasty.
Sometimes chest pain can occur during angioplasty because the balloon briefly blocks off the blood supply to the heart.
What are the benefits of a coronary angioplasty?
In most cases, the blood flow through the coronary arteries improves after an angioplasty. Many people find their symptoms get significantly better and they’re able to do more than they could before the procedure.
If you’ve had a heart attack, an angioplasty can increase your chances of surviving more than clot-busting medication (thrombolysis). The procedure can also reduce your chances of having another heart attack in the future.
Medication and further treatment
Most people need to take blood-thinning medications for up to 1 year after having an angioplasty. This is usually a combination of low-dose aspirin and one of the following medications:
It’s very important you follow your medication schedule. If you stop your medication early, it greatly increases your risk of a heart attack caused by the treated artery becoming blocked.
The course of clopidogrel, prasugrel or ticagrelor will usually be withdrawn after about a year, but most people need to continue taking low-dose aspirin for the rest of their life.
You may need to have another angioplasty if your artery becomes blocked again and your angina symptoms return. Alternatively, you may need a coronary artery bypass graft (CABG).
Cardiac rehabilitation should be offered if you’ve had a heart operation. This program aims to help you recover from the procedure and get back to everyday life as quickly as possible.
Your cardiac rehabilitation program will begin when you’re in hospital. You should also be invited back for another session taking place within about 4 to 8 weeks after you leave hospital.
A member of the cardiac rehabilitation team will visit you in hospital and provide detailed information about:
- your state of health
- the type of treatment you received
- what medications you’ll need when you leave hospital
- what specific risk factors are thought to have contributed to needing the operation
- what lifestyle changes you can make to address those risk factors
Once you’ve completed your rehabilitation program, it’s important you continue to take regular exercise and lead a healthy lifestyle (see below). This helps protect your heart and reduce the risk of further heart-related problems.
The British Heart Foundation has more information about cardiac rehabilitation.
If you have a coronary angioplasty, it’s still important to take steps to reduce your risk of having further problems in the future. This may include:
- trying to lose weight if you’re overweight
- stopping smoking if you smoke
- eating a healthy diet with low levels of fat and salt
- being active and exercising regularly
Smoking and being overweight are 2 of the main causes of heart disease. They also make treatment less likely to work.
